The 2008-2009 Membership form is now available. You can download an Adobe PDF version or a Microsoft Word (Doc) version. They are the same form, just in different formats.
Please print the form, and fill in all required fields. Don't forget the second page (Waiver of Liability). Sign the form, include a check for the full amount and then mail it to:
